How do you get it off?

I think it involves a blowtorch .. a few people have done it .. tax, nvdp5? ...

Heat up the metal with blowtorch, it is only a rubber gasget type thing that is holding it on. Place a screw driver throught the shaft and turn while heating it. Should only take you a few minutes... But be carful.. Flame in the car is not your friend.!

Question: Is the shift arm that's left threaded identically on the top as the extension?

Nope.. what is left is just a black piece of metal... if you are thinking of putting your original shift knob back on top then you have to cut off the threaded portion with a hacksaw, then file it down so that you can slide it in the top hole of the black metal shifter. If you don't understand, you will when you remove the rubber gasket. So that you do not have to do this rather tedious filing, i suggest putting a momo style shifter (with the three screws) this will just fit over the Black metal shaft and you just tighten up the three screws. This way is a lot easier! Either way it shouldn't take you too long to complete.
